Limit one entry per person, per email address, per household
Prizes: One (1) Grand Prize winner will receive a prize package consisting of a trip for two (2) to New York City including round trip coach air transportation from major airport nearest Grand Prize Winner’s place of residence to New York City, hotel accommodations (double occupancy) for four (4) days and three (3) nights, tickets to a Broadway show, dinner at a top restaurant, a makeover by a Maybelline NY makeup artist (winner only), a spa treatment for winner and guest and $500 spending money. All additional expenses associated with the Grand Prize including but not limited to federal, state and local sales or other taxes and surcharges, insurance, meals, gratuities and incidentals are the responsibility of the Grand Prize Winner and Companion, unless otherwise set forth in the Official Rules. If the Grand Prize Winner’s primary residence is within 100 miles of New York City, the Sponsor and Sweepstakes Administrator reserve the right to substitute airfare with a car service or other ground transportation to New York City. If the Grand Prize Winner is considered a minor in his/her state of residence, the prize will be awarded to his/her parent or legal guardian. The parent or legal guardian will be required to sign the Affidavit of Eligibility, Liability Release, and Publicity Release. Any minor participating in the Grand Prize package must be accompanied by parent or legal guardian or obtain the written consent from their parent or legal guardian. If Grand Prize Winner is 21 years of age or older, his/her guest may be of any age, however, if his/her guest is considered a minor in his/her state of residence, such guest’s parent or legal guardian must provide written permission to accompany Grand Prize Winner. Travel must be completed by 7/31/07. Travel restrictions and black out periods may apply. If the Grand Prize Winner is unable to travel to the location on the dates specified and agreed to, the prize will be forfeited and another winner may be chosen. Approximate Retail Value of the Grand Prize: $4900
Three Hundred (300) First Prizes: Each first prize winner will receive a Great Lash cosmetic case filled with a collection of Maybelline NY products. (ARV$25)
