I finally managed to get the survey to pop-up. I answered 2 questions & it froze on me. When I went back to the site - no pop-up again. UGH, it is one of those days for me.

Wow, I just took the 2nd part of the survey after visiting their web site. It was also easy. 3-4 weeks before they e-mail the gift certificates.
 
I turned off the blockers, but so far I can't get any survey for it?
 
don't choose the option "health" field (I think it was option #1) when it asks if your immed. family works in any of the professions. it ended the survey for me after i answered that.

hmmmm.......i have have high cholesterol and I got it after admitting to it LOL after saying that it was unlikely that I would ever use crestor was when I thought it would boot me out!!
