I am sorry guys, but I don't remember all of the answers. I do remember putting that I used semi permanent color and the last time I colored my hair was over 4 weeks ago, but within the last 3 months. I am also in the over 30 age group range. I hope that helps!

I filled it out using my mama's info 57 semi-permanent colored hair in 4weeks to 3 months. I got booted for me picking highlights and backspaced and I got booted for picking the color that wasn't there.... Hope it helps!
