There's a couple of things here that confuse me:

1. The rebate form says "Limit 3 rebates per product, per address". I assume this means you can get rebates of up to 3 purchases of any and all of the items?

2. This is REALLY confusing. The rebate form says "If multiple products are purchased on the same order, consumers should submit a copy of 1 rebate form, 1 packing slip and a copy (or copies) of UPC(s) to....." Does anyone understand this or am I the only one confused?

Not to long ago I purchase 3 goodie boxes from amazon.com and the wording on the rebate form was exactly the same, limit 3 rebates per product. I submitted 1 rebate form, 1 receipt and the 3 upc, clearly noting that I was requesting 3 rebates. They sent me 3 separate rebate checks, all received on the same day.

Same here. I include a very brief, polite cover letting explaining that I purchased 3 copies of xyz item and I circle the quantity on the packing slip and highlight it to further point it out.

You can do what you wish, but I like to scan the UPC codes, then paste them into a new image in any of a number of cheap, entry level photo programs. I use iphoto back from 1997 I swear, but paintshop pro shareware would work. Above each UPC I letter what the UPC is for, and print out one page with all the UPCS. If I bought 3 copies of a given title, I will paste in the UPC code three times and label it "so and so UPC #1" and "so and so UPC #2" etc.

Seems to work for me.

Anyway, all they mean is, don't send a separate submission/envelope for each rebate you are entitled to, but just print out and fill out the rebate form once, one copy of the packing slip, and include all copies of whatever UPCs for the products you bought with the one submission. FYI, you very much can use UPC copies, so you can buy stuff like this as stocking stuffers without having to cut out a UPC and disturb the plastic wrap.
