This to me does not look like a legitimate site. I would be very careful giving up all the information they ask for here. They even ask for your social security number.

The site does not even look like a company's website. The only place you find any reference to this site while searching Google, is in Yahoo groups and MSN groups. And even those pages are dead.

There is no company address, no company phone number. The site advertises the Trust-E privacy seal but nothing in the webpage is secure, not even where you have to enter all the information. Also why does it state "your IP address will be sent with this form", what does that have to do with anything.

I agree. Good find Choochoojr. If a site is asking for your social security number and it isn't some type of official site, I don't think it should be offered here! Good find, too bad it wasn't real. Thanks for catching this before I went for it!
