NOTE: Must have been to the pediatricians office recently (within 3 days) and don't say a friend referred you.

Thank you for visiting our online research community. You can help us decide the best ways to share information on childhood illnesses, medications and health education. Most importantly, we won't share your name or responses with anyone else.

This survey is designed for parents or guardians who have visited their child's pediatrician within the last 3 days.

If you qualify for, and complete the survey you will be paid $5.00 for your time. Thank you.

Must be the parent, grandparent or guardian of the child who visited the pediatrician and complete the survey within 3 days of visit to qualify. Only one survey per household will be accepted. Your participation is completely voluntary.

The survey should only take about 15 minutes to complete. To advance through the survey questions, just click "Next" located at the top or bottom of each page. For your convenience, you can also check your progress by looking at the "progress bar" at the top or bottom of each page.

Rats, I was almost completely done with this survey, and now when I click the next button it says its not available......:(
 
I completed this survery about 10 minutes ago and everything worked just fine.
 
I made it all the way through to the page where it asks for your dr. phone number. I looked it up and entered it in, then when I clicked next, the survey said not available. please don't tell me that was the last page and I did all those questions for nothing. lol.

I hate to say it but that was the last page. After I submitted mine it said that I would be compensated the $5 for completing the survey.
 
It worked for me too...I made it all the way through and said I would get the $5, BUT I kept getting the page unavailable too and I just used the "back" button or the "refresh" button and submitted that page again. I actually got it a few times throughout the whole survey.
