Anyone had any success with this?

The signature / authorization line makes it seem like they are going to get spammed with these faxes and then no one get anything.

Any suggestions or ideas?

I moved my cursor over the link to see where it would go and yep there's no way I will go there while at work! I didn't know they had a penthouse club in Tampa. I wonder if this is a special promotional deal for a new club. Enjoy folks.
 
My husband and I sent off for this a few months ago using this fax in form. We started getting the mag last week. We didn't add any kind of authorization number or anything in that blank for that matter. Also, I don't think there is a club anywhere near here. As for spam, I always use the same email when signing up for something like this so I have no idea if I get more because of it. This does come though. :)
