I hope I get this one, had no idea how to answer those questions so left them blank (like what brand etc do you use? )

I filled the form out again, because I guess because it was back in 11/07,
I don't know or remember if I filled out the form totally. LOL LOL
 
Never received it so tried again! Hopefully I'll get it! Made sure everything was filled out in full.
