Free Golf Balls from Michelob - Select States (new link, no purch req)

I could not get the form to download. Tried three browsers, and they all gave "the page cannot be found" message.

says for florida you need to provide proof of purchase! That stinks! :*(
Ok sent my mom a set of golf balls as she lives in NY and can then send them to me! :)
 
Well, I still can't get thru--I did get a page to enter my birthdate and state, but after I enter those items, it just sits there. There is no "continue" button. This one is weird (at least for my computer).

Thanks--a lot of trouble for some balls! And we don't even play golf. But, for $.44 postage, I can add them to my garage sale stash or gift them to someone.

Ok so why are we discriminating against half of New England... It is usually all or nothing with us... MA, ME, CT - no NH
 
I requested these for myself and for a friend--she received them last week (nothing here yet). So, after all the angst, I guess I did something right.
 
Not valid here in FL either but is Valid in NY so sent it to my mom to give to a friend of hers who plays golf! :)
 
ok the thing says must be received by june 21 2010! WTF? Then the website says July 6th..must've extended this i think!
 
I was able to get the first link to come up but it said had to be received by 6/21/10.
