I realize this compnay is in the UK, but two weeks ago I submitted my request and the cookbook came yesterday so it seems they will honor USA requests.
listed my address this way:
First Name*........First name
Surname*...........Last Name
Address 1*..........Street Address

Town/City*..........USA
Postcode*..........Zip Plus Four
They sent it airmail too!
