Last time I ordered the chair + mat combo from OD. The delivery did not come with a receipt. The rebate I sent in with the email receipt is currently considered incomplete. I'm still trying to get a copy of the receipt from OD. :?

I got the deal on 7/11 and went to print out the rebate, but the one that currently comes up requires a purchase between 7/20 and 7/26! Anyone have a link to the original rebate form (the one in the initial post doesn't work anymore).
