If one looks at the rebate form, it will say "Dominator in 1 Dual 4X DVD+/-RW" at the top.

At which point, one might say "but the drive being sold by officemax isn't a dual drive, it's just a -r/rw dvd burner, is this the correct rebate. The key is in the UPC number listed in the rebate form.

#1 - Mad Dog doesn't sell a 4X 6-in-1 dual format burner. They sell a 4x 6-in-1 -r/rw drive, but the next drive up from that in their product line is the 8X dual format +/- drive, which is the NEC 2500A

#2 - The UPC number listed on the rebate form (833054001552) coincides with the 6-in-1 4x dvd-r/dvd-rw drive, as seen here:

#3 - Even Mad Dog itself says that UPC # 833054001552 is their 6-in-1 4x dvd-r/w drive as seen by this older mad dog rebate from november of 2003

So I'm reasonably confident this is the correct rebate, just someone messed up when saying dual format drive in the rebate form.

If you have a 10 off 50 in store coupon, this could be $9 AR.

Clearly associating that UPC # with their 4x -r/rw dominator drive.
