To receive your mail-in rebate:
1) Purchase the specially marked Physicians Formula® Mineral Wear™ Talc-Free Mineral Foundation
product between 1/1/06 and 6/30/06.
2) Print out and mail this completed form along with the TRY ME FREE label attached to the product
and your original sales receipt with purchase price circled.
3) Rebate must be postmarked by 7/15/06.
 
Is Physician's formula found at stores like Longs, WalMart or at a Department Store?
 
I've seen it WalMart, Target, and most drug stores. They make really great products.
