Autozone - Up to 5 - 1Qt. of Valvoline MAXLife Oil for Free After Rebate

Autozone has a new rebate form available for the 5qts of Valvoline MAXLife oil. (Note: the rebate form should be available in the store.) It's for cars with over 75,000 miles. Autozone carries 1qt. of Valvoline oil for $2.69. Rebate is good up to 5 qts. and up to $15. The rebate form must be original and can be picked up at the cash register.

The rebate offer will expire on June 4th. Find an Autozone near you.

can someone direct me to this rebate form? Having problems locating it on the site!
 
I went to AutoZone today and bought the MaxLife but I couldn't find any rebates on it. If I can't find a rebate online, I am taking it back.

That's not for AutoZone. That's for Advance Auto Parts. Where is the AutoZone rebate?

OK, i've called two local AutoZone Stores, and they both noted that the form IS available in store. unfortunately, they don't have fax machines so they can't fax it to me.

Anyway, the form should be available instore. If your store didn't have it 91bear, then i'm sure that some kind person will scan it & post it so you'll be able to submit your rebate.

The attachment says no copies accepted. The original rebate form must be included. I have been to two AutoZone stores here and no one knows anything about the rebate. How can I get an original form?
 
Are you sure this is for AutoZone and not Advance Auto Parts?

it is for AutoZone.

Afraid not, the rebate form states that you need to submit the original rebate form

Also, most Valvoline products have two codes each on the back of the bottles, so when you enter six codes from three bottles on the website you get a "Daytona International Speedway" or "Bristol Motor Speedway" tshirt or hat of your choice.

By the way, the papers on the back of the bottles are almost all already torn open from bad production and easily removable, so I would try to get the bottles from the back or open a new pack in case someone else already copied the codes or removed the papers.

This isn't an Autozone promotion, so you can check it at other stores that sell Valvoline products too.

please check Spoofee's first post. Click on the images below where it says "attached thumbnails". those are the scanned images for this rebate form.

as you probably already know, you need to submit an original rebate form to qualify for this rebate. if your local autozone store doesn't have it, i think it might be worthwhile to call around & see if there's another Autozone that would be willing to mail it to you (you could even offer to send them a SASE)

if that fails, try calling 1800AUTOZONE to see if their customer service dept can help you out (although i've already called them at least 3-4 times, & they kept transferring me to random people's voicemails)

Hey Everyone, i just went to AutoZone again today and they had the rebate forms in. They said that they just got them this morning, so you may want to call your AutoZone again if they did not have the rebate forms the first time. Hope this helps.
