hm, have you tried clearing your cache and then checking the webpage? if that doesn't work, you might need to delete your cookies and then it should work.
 
This is my first time using your sight I just signed up with you, I went to sign up for the free sample of dog food, after filling out there form ,they stated Sorry but you have exceeded the limit for form submissions today previous malicious activity we have instituted this limit to better se our customers. I replyed to them stating that I had not submitted any froms to them or anyone else today or ever, that this was the first time I had reguested a sample from anyone, and that I did not appreciate them suggesting that I was doing malicious activity. I thought you might like to Know this.
 
Odds are they're filtering ip domain ranges which they feel are more prone to attack or take advantage of them. I'm not a mod so can't see your guy's ip addresses, but I'd be willing to bet the first 2 sections are the same. This is just my guess..

You can always hunt down a public proxy and run the website via the proxy to mask your ip for submission which should work if that's how they're 'defending' themselves...
