I believe you can use three of the same one. Don't try to photocopy them though.
 
I collected the POPs, but I just found that the form is no longer available, even though I'm pretty sure the expiration date has not yet passed. Did anyone print it out? What is the thinking on why the deal was pulled (and whether we'll get the DVDs)?

Maybe they ran out of DVD's I do believe the form stated while supplies last.
 
I was wondering did anyone yet receive their Hulk DVD? I know they cashed my $3 dollar check but I have yet to receive anything in the mail. Just wondering what the status was.
