Promotion begins at noon ET on November 27, 2007, and ends at 11:59 P.M. ET on January 17, 2008 ("Promotion Period"). Beginning at noon ET on November 27, 2007, an interested beauty tester can request Healthy Beauty Awards products by visiting Self.com and clicking on Be a Beauty Tester, then completing and submitting the form by 11:59 P.M. ET on December 7, 2007. The form will require her to electronically sign a release of liability and fill out the following information, all of which will be returned via online submission, to be considered: name, address, phone number, e-mail address, age, skin type, hair type, texture and color, types of hair styling and anti-dandruff products used, and indicating whether or not she currently colors or highlights her hair and/or would be willing to color or highlight her hair. Approximately five (5) beauty products (while available) will be sent to sample (approximately 800: exact number to be determined by Sponsor) of respondents who are appropriate candidates for certain products based on the skin and hair information provided. Selection of testers will be on a first-come, first-served basis. The products will be sent around December 15, 2007, depending on the number and speed of responses.

Help choose the 2008 Healthy Beauty Award winners!

If you're at least 18 years old, you can enter to test-drive some of the latest hair, skin and makeup products. Once selected (nearly 800 testers will be chosen on a first-come, first-serve basis), you'll receive a sample of products along with instructions for completing an online tester questionnaire.

Form is up now

The form is accepting now and it does not say anything about Nov 27. I was chosen last year and I received 8 different tubes of very expensive lotions to try. One bottle was over 100 retail. I hope to get chosen again.:)

After being selected as a tester, you may enter the sweepstakes by testing the products and completing the online Healthy Beauty Awards questionnaire. Alternatively, testers may enter the sweepstakes by mailing a postcard between 3 1/2 inches by 5 inches and 4 1/4 inches by 6 inches with their name, daytime phone number and e-mail address to SELF Healthy Beauty Awards Sweepstakes, 4 Times Square, 5th Floor, New York, NY 10036. All entries must be received by 11:59 P.M. ET on January 17, 2008. All mail-in entries must be postmarked by January 17, 2008, and received by January 22, 2008. Entries that are lost, late, damaged, illegible, incomplete, incorrect, misdirected, postage-due mail, garbled or incompletely received for any reason, including by reason of hardware or software browser, or network failure, malfunction, congestion or incompatibility at Sponsor's server or elsewhere, will not be eligible. Sponsor, at its sole discretion, reserves the right to disqualify any person tampering with the entry process or operation of the website. Use of bots or other automated process to enter is prohibited and may result in disqualification at the sole discretion of Sponsor. Sponsor further reserves the right to cancel, terminate or modify any promotion not capable of completion as planned, including by reason of infection by computer virus, bugs, tampering, unauthorized intervention, force majeure, or technical failures of any sort. Mechanically reproduced entries are not eligible. Only one entry per person. The approximate retail value of the prize package is $750.

I am wondering if I can sign up for the Beauty Product Testing today since the form it up?

I do not want to be disqualified for filling out the form early but also do not want to miss out on this.
