Adidas sends you an email after you enter your info on the shoe lace request page, but i noticed that i couldn't use the same email "addy" to enter add'l info (like mailing address). So here's what I recommend:

You can register first here: http://www.adidas.com/com/shared/register/reg_sign_up.asp
and under "optional", provide your mailing address.

then go to the shoe lace form:
http://www.adidas.com/us/register/a3axiom/register.asp

(I recommend using the same email address and other information)
