It would have been neat if this had come in time for the NALC Stamp Out Hunger food drive this Saturday. Don't forget to leave a sack of non-perishible food items at your mailbox to be distributed to local food banks. :)

Yes, there is a form--but have fun finding it. I had a hell of a time trying to get to it and still don't know what I clicked on which finally brought me to the right page. But I did get through tonight. Thanks.

Oh didnt think about that, for anyone else after you click the like button, click on the libbys table tab, then it should say click here to fill out the form, hope this helps anyone else signing up :)

That's the way it worked the first time I went there, but since I had already clicked "Like," I got different pages the second time. But I wasn't easily discouraged, and finally got the form back. The direct link is so much better. Just signed up my friend. Thanks, hoffmann2828.
 
Check you emails. They are sending printable coupons. Mine was in my SPAM folder.
