I think you have to be logged in to your facebook account first even tho the form comes up with the link above. I filled out the form and then it directed me to log in. I logged in, the form came back and I was able to submit.

I got the same thing so then I read what Pat137 said and logged into facebook. , filled out the form and it said I had already submitted. Guess I'll know when/if it comes:prey:

I was already logged in, I completed the form and after submitting it brought me back to a blank page too so I guess I will wait and see!

Make sure you click to become fan.Samething happened to me but then i went back to become fan!! HTH!!! Thank you glitterdog:clap:
 
I filled out form,and then I submitted and the page came up again.will see.
 
You must "become a fan" and be logged into your facebook for the form to properly submit.
