(Expired) Free Saladworks Shirt

How to Enter: To receive prize, promotion participants need to visit http://promo.saladworks.com/shirt and fill out all required fields on the online form. Upon completion of form, Saladworks will mail a checked shirt to participants. Available to first 1,000 people who complete the form on or before October 8, 2010. Upon receipt of prize, participants are encouraged to take a photo of them wearing the checked shirt at a Saladworks location and share that photo with Saladworks on Facebook (http://www.facebook.com/saladworsk) or Twitter (http://www.twitter.com/saladworks). One entry per promotion participant. The Promotion begins at 9:00 a.m. Eastern Standard Time ("EST") on September 20th, 2010 and ends at 12:00 p.m. EST on October 8th, 2010 (the "Promotion Period").

Awarding of Prizes: The first 1,000 people to enter the promotion are guaranteed a checked shirt. The entrants will be prompted by an on-screen notification confirming their request. Winners will receive the shirt within 4-7 days of successfully submitting their information via the designated online form.

Prize (1,000): Sponsor will award a total of one thousand (1,000) checked shirts.

Received my shirt today UPS in a big, white envelope (shipping slip indicates it's worth $15). Came from Flagstaff Industries in PA, Ref. #798994C.
 
even thou I replied intime and got the right response for sumbitting. I later got an email saying I would not be getting the shirt.
 
I must say...I look pretty snazzy in my orange checked shirt.

I signed up for this and got a confirmation that I would be getting it in the mail.... hopefully it comes soon! It has not gotten here yet.
 
Both my husband and I got our shirts, and I took them with me to the mall at Willowbrook yesterday (I didn't wear one). The manager knew nothing about the promotion, but he did give me two free salads even though I was a day early, didn't wear my shirt, didn't have my husband with me (although I did go with a friend), didn't want tomatoes and asked for a substitute, and just generally shook up his day! He said I was the most interesting customer he ever had. And then he told me not to come back on Friday wearing my shirt!
