After answering the survey, we end up on a page where there is a link to get the T-Shirt. Make sure you click on "Order Sunism Gear " after completing the survey.
After clicking on the link, they explicitely confirmed that I would get the T-shirt:

"

All's I'm saying is that they ALWAYS promise you the t-shirt, but more often than not deliver you :)

I have better odds in Vegas than getting a free shirt. Maybe I'll hit it big this time?!?!?!

The shirt does come. I filled out for one a while back and got it.
 
I am a big guy. I hope they don't send me some Large T-Shirt....because they never did ask ;-)
