Worked for me, thanks. It took a while to "reload" but them a form came up and I was able to enter my info. It "timed out" when I pressed submit but after I re-loaded again I got a screen that popped up and said I was "in" and would be receiving a dvd.
 
nothing's loading...assuming gone by now.

Edit: Nevermind they just pulled the form, said I was going to get one after the form went thru??
 
ok...got the form finally. Says "You're in. We'll send an email when your DVD is on the way." Here's hoping. Thanks for the post
