UAB Cancer Answers Program - Green Lunch Cooler

Click on registration form, bottom middle of page. Sign for a free duffle bag (black) and a free cooler bag insulated-type (green) from UAB Hospital. It doesn't tell that they will send you these items but 2 weeks after I signed up for cancer info and heart info I got these. And they were priority mail, they paid $4.20 for each just to mail it them to me. It doesn't matter if you are from another state they get patients from all over US. Nice gifts.
 
It only seemed like 1 survey was available. Do they send you both items?
