This site asks for the info it does because you must be 21 or over to request this freebie because it is from a tobacco company. This is the same info they request for other various tobacco freebies. :)
 
thanks, this seems similar to the Marlboro prize game they recently had. And this isn't a scam, I had to enter the same info when I signed up for the Marlboro site.
