I forgot to mention you need to choose that they contact you by mail and you will get the form for your addy. Sorry About that....

Went back and did mail after I had already done email only.. it let me.. hope I get it.
 
I got my sketch pad in the mail today. It is very nice. Thanks for the posting.
 
They ran out of sketch pads and sent me Oakley stickers. My son liked them, so I'm happy!
