Step 1: Use "..." for the Insurance Provider Drop Down Box.

Step 2: Choose the monitor of your choice:Ultra 2 or One Touch Ultra Smart.

Step 3: Enter your contact information. Where it asks for the BD Monitor number, use: 1-888-232-2737
Lastly, it gives you a confirmation: "Thank you for selecting a new OneTouch® Blood Glucose Meter. You will receive your new OneTouch® UltraSmart® Meter and Simple Start™ education materials within 3 business days. "

Thank you so much! This is really nice for medical professionals to give to patients who can'g afford the monitor, or, who do not know they even exist. I have a few friends and family members who have diabetes. This will help a lot! :wave:
 
when i entered the insurance and when i selected the ulta smart option and pressed submit i got this message .

I got an E-mail from One Touch today saying that my meter was shipped and I should be receiving it soon. :bigok:

Free Meter

Got mine yesterday:) . These cost about $100 near me:mad: . I am very grateful for this one. I wish I knew of someplace to get the test strips for free. They cost about $90.00 as well. Can't afford diabetes without health insurance, thats for sure.
 
yeah i know...now that I have the monitor, i need to find a good deal or freebie on test strips.

For those getting this message, in the section right below where it says "Following information is required if you are NOT a Health Care Professional"...either check the box next to "This is my first blood glucose monitor" or select any of the monitors except the One Touch ones listed in the small window below that. After you hit submit on that page it'll take you to Step 2 where you choose the monitor you want.
