Worked great for me... :) Make sure you have all of the fields filled in and marked that need it.
 
Whooops....my bad.....I forgot I had my security settings on high.....the survey works fine.

I'm having trouble with this one. Nothing happens when I click next page. Eveerything is filled out already. :confused:
