Method #1: Visit
www.freschettasweeps.com and follow all entry instructions to fully complete and submit the online entry form ("online entry"). Online entries must be received during the Sweepstakes Entry Period. Online entrants subject to all notices posted online including but not limited to privacy policy. LIMIT: One (1) online entry per person or per e-mail address per day. In the event of a dispute over the identity of an online entrant, entry will be deemed submitted by the "Authorized Account Holder" of the e-mail address submitted at time of entry. Authorized Account Holder means the natural person who is assigned to an e-mail address by an Internet access provider, online service provider, or other organization that is responsible for assigning e-mail addresses for the domain associated with the submitted e-mail address. Potential winners may be required to show proof of being the Authorized Account Holder. Entry materials/data that have been tampered with or altered, or mass entries or entries generated by a script, macro or use of automated devices are void.
Prizes & Approximate Retail Values ("ARV"):
One (1) Grand Prize: Four (4) day/three (3) night trip for two (2) to an independent film festival ("Event") in Park City, Utah (ARV: $3,396.00). Actual ARV depends on location of winner and fares at time of booking. If ARV turns out to be less than the stated ARV, the difference will not be awarded in cash. The Grand Prize consists of and is limited to: round-trip coach airfare for two (2) to/from a major commercial airport nearest to winner's primary residence (carrier and seating as determined by Sponsor), three (3) night hotel stay (hotel and double occupancy room as determined by Sponsor), two (2) tickets to four (4) movies at the Event (total of 8 tickets; seating, movies, dates and times as determined by Sponsor), two (2) ski lift passes for one (1) day (resort and date as determined by Sponsor), two (2) tickets to one (1) festival party (date and time as determined by Sponsor), ground transportation between airport and hotel (as determined by Sponsor), and $100 total in spending money. All tickets and lift passes subject to the terms and conditions thereof. Taxes, gratuities, additional ground transportation (including, without limitation, to and from the originating airport), additional lodging, insurance, and any other expenses and incidentals not expressly stated above are the sole responsibility of the winner and guest. Winner and guest must travel on the same itinerary and on dates and at times specified by the Sponsor or forfeit all rights to the Grand Prize. If the Grand Prize is forfeited, and there is sufficient time to notify and process an alternate winner, the prize will be awarded to an alternate winner in a separate random drawing among all non-winning eligible entries in the Sweepstakes. If the Event or any portion thereof is postponed, canceled or has occurred, neither Sponsor nor Administrator shall have any further obligation to winner other than to fulfill the Grand Prize minus the affected Event tickets portion of the prize, for which no replacement compensation will be awarded. Winner and guest are responsible for obtaining proper documentation (e.g., valid photo ID) prior to travel. Grand Prize winner's guest must complete and return Sponsor's Liability/Publicity Release prior to travel. If the Grand Prize winner's guest is a minor, then s/he must be accompanied by his/her parent or legal guardian and the parent or legal guardian must complete and return Sponsor's Liability/Publicity Release on the minor's behalf prior to travel. Sponsor or Administrator will file an IRS Form 1099 with the Internal Revenue Service with respect to the Grand Prize winner for the retail value of the Grand Prize. The potential Grand Prize winner is required to provide his/her social security number for tax reporting purposes.
Twenty-Five (25) First Prizes: A First Prize consists of and is limited to a collection of five (5) DVDs (as determined by Sponsor) ,one (1) coupon for a free Freschetta™ frozen pizza (coupon subject to its stated terms and conditions) (ARV of each First Prize is $59.00, and the total ARV of all First Prizes is $1,475).
