memory foam mattress covers

I want one of those memory foam mattress toppers really bad cuz I've been seeing deals for them for quite a while but they always seem to run out of stock. First I want to know how you're supposed to use them.
Are you supposed to just put them on top of your mattress and them cover them together with together with the same fitted sheet? Or do you put it on top of the mattress over the fitted sheet? And how do you clean them? Is there a cover for the mattress topper itself that you should buy?
