Nature Made Rx Essentials (free after rebate)

To receive your full rebate by mail:

1. Visit your local retailer and purchase Rx Essentials. Don’t forget to save the receipt!
2. Print out the downloadable rebate form and fill it out.
3. Clip the UPC symbol (bar code) from any carton of Nature Made’s Rx Essentials product line. Do not staple, glue, or tape the UPC to the refund form.
4. Along with the completed rebate form, mail the UPC and original cash register receipt dated between 12/3/07 and 3/31/08 with the purchase price circled to:
