TRADE IN PROGRAM

On Earth Day (April 22, 2012), customers are invited to "trade in" any one of their existing skin care products, regardless of brand for an Origins high-performance moisturizer, while supplies last at Origins retail stores and department store counters for FREE. Customers will have the choice between A Perfect World™ SPF 25 Age-defense moisturizer with White Tea and Starting Over™ Age-erasing moisturizer with Mimosa. All cosmetic packaging collected will be recycled or responsibility converted to energy through the Return to Origins Recycling Program.

The form is from last year. When will this years form be posted? We are getting closer to the 22nd!
 
Thanks! Is anyone else having problems printing the form . . . it cuts off the part where you fill it in and does not show the zip code, etc on the far right.
