Welcome to the Federal Student Aid (FSA) Product Awareness Survey. FSA is a division of the U.S. Department of Education (ED).
This survey is being conducted by UserWorks, Inc. on behalf of FSA. The purpose of this survey is to gather information about your awareness of FSA's informational materials about financial aid for education beyond high school. If you have any questions, you can contact us by email at

You will first be asked a few questions to see if you qualify for the study.
If you qualify and then complete the survey, you will be awarded a $10 gift certificate from Amazon.com. The survey takes about 10 minutes to complete. You can only take this survey once.
This study is voluntary and you may stop at any time; however, you will only receive the $10 gift certificate if you complete the survey.
Any information you provide will remain completely confidential; we will not give or sell your information to anyone else. You will not be asked to provide your name; however, we will ask for your email address in order to send you the $10 gift certificate. You may complete the survey without giving us your email address, but we will not be able to send you the gift certificate in that case.
