I don't see the survey anywhere. Am I missing it?
 
I looked a little while ago and didn't see it either. Must be gone now. Cute avatar, kbabe!:)
 
It comes up as a pop-up so if it's not coming up it's probably expired.:wilted_ro
 
It's a pop-up. Turn off your pop-up blocker
