products using the same rebate form ... send in one or several of the same form?

I have several products using the same rebate form. Should I send in one form highlighting products or many forms each highlighting it's specific product. The former I think my confuse them associating products with receipts, and they won't send my rebate. The latter, they say, "You sent us many forms, when you could have fill out one. No rebate for you!".

What do I do?
 
I guess just do what the reabte form says. Does it say submit each rebate seperatly, or anything about multiple rebate submissions. Most places that have a rebate form for several differnet things, you just check the boxes of the items purchased and send in the receipts. But it should say for sure on the rebate form.

Where is this for, is there a copy of the form available for someone to look at and see for sure.

I think your suggestion is the way to go, whatever the form says.

I think in the past I may have gotten screwed or something, that's why I'm asking before I send in the rebates. (I am however tracking my rebates now, previously I didn't, using excel spreadsheets. I might post it someday, in case others might be interested for ideas, or can give me ideas for improvement).

The form above suggest I can send one form for many products, so that's what I'm going to do.

I would submit that form above with the UPC codes and receipts all together. If you look at line #5 it states you can do that.

Just make sure you make copies of everything you send in, that way if you need it later you will have everything. :)
