Sky, Sam's Club and Walmart are owned by the same people so it makes sense. At the end of the form it asks you if you have received a free sample from Walmart in the last 6 months for this product.

I got a sample of this from Walmart. It works. It works amazing actually.
 
Rcvd my 28 tablets sample but didn't get the pill box. Actually was looking to get the pill box. Wonder how it works. Oh well. Still a good freebie.
