I printed the form from here ... http://www.circuitcity.com/rpsm/oid/166345/rpem/ccd/RebateDetails.do .. I couldn't find the rebate form on the website either, but I don't think that's a reason to doubt the rebate. It clearly states "QUALIFYING MODELS: $30.00 Mail-In Offer on purchase of SDK SDMSPD256A10 Qty: 1 Rebate Amt: $30.00"
