Settlement claim form for people charged casual caller rates between March 97 and December 03, fill out form by October 21, 2004 and you must include your phone bill.

That's kind of rough for people who disconnected recently. I don't know of any people that keep a phone bill for several years. It looks like the phone bill is required for the $100 phone card instead of the $50 phone card.
 
Is this ONLY for Sprint Landline? Or also Sprint PCS / Wireless?

How can I tell if I was charged casual caller rates?
Your past phone bills will not specifically state whether you were charged casual caller rates or surcharges. (In the telecommunications industry, these are also referred to as "casual calling," "dial-around," or "random" rates and surcharges.) Any such charges would appear on bills received from your local telephone service provider, on a separate page identified as charges for Sprint long-distance service, and would reflect higher rates and surcharges than your Sprint rate plan. Sprint imposed casual caller per minute rates of $.40 per minute and a per-call surcharge of $1.99 on domestic calls. If your phone bills reflect that you were charged approximately $2 for a one-minute phone call, then it is likely that you were charged casual caller rates.

The way i read it, you only need a phone bill if you want $100. I don't have a phone bill...so, I went for the $50 card ...
