If anyone had trouble getting the unique code to fill in automatically, I read Wall posts that said you need to turn off the Secure Browsing feature on facebook (Account Settings > Account Security > Uncheck box for "Securing browsing (https)").

Then just go back to the Indochino page (the website address should now start with "http" not "https") and click on the link for the free tape measure.
